SWEET HOME, Ore. - A Sweet Home man died after his van "for an unknown reason began swerving back and forth in both lanes of travel" and collided head on with an oncoming SUV, Oregon State Police said.

The crash happened around 2 p.m. Tuesday on Highway 20, east of Sweet Home.

The investigation found that a 2001 Pontiac van driven by Scott A. Wartena, 76, of Sweet Home was westbound when it began to swerve.

Police said Linda K. Phillips, 69, of Boring tried to avoid a collision but her Ford Explorer collided head on with the van near the fog line of the eastbound lane.

Medics took Wartena from the scene. He died from his injuries Wednesday at a hospital, police said.
